CIHR at a Glance

The Canadian Institutes of Health Research (CIHR) is the Government of Canada's agency for health research. Its mandate is to create and translate new knowledge to help ensure improved health for Canadians, more effective health services and products and a strengthened health care system.

Through its 13 institutes, CIHR is creating new opportunities for Canadian health researchers to produce results that matter to Canadians and the rest of the world. CIHR uses a problem-based, multi-disciplinary and collaborative approach to health research. The majority of its funded research is investigator-driven, while other funding is for strategic initiatives that respond to health challenges and scientific opportunities of high priority to Canadians.

CIHR's approach is enabled by its structure - unique in the world - that brings together researchers from across disciplinary and geographic boundaries through its 13 Institutes, each of which addresses a theme of health research of immediate and identifiable importance to Canadians.

President's Message

In 2003-04, Canadians truly witnessed the exceptional value that health research offers.

SARS tested the ability of Canada's health researchers to act quickly and decisively to meet a health crisis - and they showed they were up to the challenge. Researchers in British Columbia and Manitoba, working together, took just 11 weeks to sequence the SARS genome. By December 2003, Canadian researchers announced that they had developed not one, but three potential SARS vaccines, ready to test in animals. At the same time, other CIHR-funded researchers announced that they had discovered a potential treatment for SARS, using a combination of steroids and interferon.

It was also a year in which health researchers made a huge stride forward against E. coli 157 - the bacterium responsible for the Walkerton water crisis of the summer of 2000. A vaccine developed by two CIHR-funded researchers - Dr. Brett Finlay of the University of British Columbia and Dr. Andy Potter of the University of Saskatchewan - was shown to be effective in cattle. It is now being marketed worldwide by Bioniche Life Sciences, a Canadian biopharmaceutical company. By vaccinating herds of cattle and preventing E. coli from entering the food supply, we can reduce its toll. Each year E. coli makes 50,000 North Americans ill, kills 500 and costs the economy an estimated $5 billion.

As this example shows, the exceptional value of health research is found not only in better health and a stronger health care system tomorrow, but also in a growing knowledge-based economy today. CIHR has a coherent suite of programs to help take discoveries from the laboratory to the marketplace. Throughout 2003-04, CIHR expanded its dynamic and innovative commercialization strategy to further enhance our ability to mobilize research, develop people, and build partnerships to help bridge the gap between ideas and new products and services.

2003-04 was also a transformative year for CIHR. In our first three years of existence, we laid the foundation for an energized and innovative health research enterprise for the 21st century. In January 2004, we released Investing in Canada's Future: CIHR's Blueprint for Health Research and Innovation, a strategic plan that articulates CIHR's vision, mandate and strategic directions for the coming years.

During the summer of 2003, CIHR conducted national consultations to gain direct input from health researchers and other stakeholders across the country. These consultations built on the extensive work of our 13 Institutes in identifying their individual research priorities, as well as on the partnerships that CIHR has forged with researchers, universities, hospitals and other health organizations, governments, research agencies, voluntary organizations, industry and the public.

With Blueprint, CIHR is poised to move to its second stage of evolution - a stage that is designed to accelerate the pace of discovery and its application, ensuring that Canadians continue to reap the exceptional value of health research with improved health, a strengthened health care system and a growing economy. In the pages that follow, I invite you to witness how CIHR and the research it funds is providing exceptional value for Canadians.

CIHR Update and Overview

Throughout 2003-04, the Canadian Institutes of Health Research (CIHR) continued to demonstrate the exceptional value realized by the federal investment in health research.

Since 2000, when CIHR was created, more health researchers are being funded with higher levels of funding in more disciplines and in all provinces. The result is health research discoveries that are making a difference to the health of Canadians, in terms of preventing - and improving diagnosis and treatment of - disease. Other research discoveries are helping to strengthen our health care system to better meet the needs of Canadians wherever they live and whatever their situations. Still others are resulting in innovative products and services offered by Canadian companies, thanks to the assistance that CIHR offers to health researchers in taking their discoveries from the laboratory to the marketplace.

In 2003-04, CIHR launched Investing in Canada's Future: CIHR's Blueprint for Health Research and Innovation, a strategic plan that builds on CIHR's first three years and charts a path for the next phase of growth. The plan sets out five strategic directions for CIHR. This annual report demonstrates how CIHR is already addressing these areas in ways that matter to Canadians, as well as some of the early results.

Strengthening Canada's Health Research Communities

Progress in health research demands the best researchers, supported at internationally competitive levels of funding, working in stimulating and supportive environments. The results are clear: research discoveries that translate into improved health and a stronger health care system and outstanding researchers who train and mentor the next generation of health researchers and care providers.

Canadian researchers are among the most highly respected in the world. In all fields of R&D, Canadian scientists are ranked 2nd in publications per researcher, citations per researcher and citations per unit GDP, across G8 nations.

CIHR is fostering and strengthening excellent health researchers in robust environments. In fact, 94 cents of every dollar CIHR receives from the federal government goes directly to supporting health researchers through grants and awards. About 77% of its research budget in 2003-04 went to investigator-initiated funding in all areas of health research, while 23% went to support strategic initiatives to address the health research priorities of CIHR's 13 Institutes. This is close to the 70/30 target set by CIHR's Governing Council as a goal that would support research arising from an investigator's own curiosity and belief in its importance (a belief supported through a rigorous peer-review process) while also responding to the strategic health priorities of Canadians.

In 2003-04, CIHR funded nearly 5,000 operating grants; the average value of open competition grants awarded in 2003-04 was $106,000. In total, CIHR supported the work of more than 8,000 health researchers throughout Canada. In addition, it supported the next generation of health researchers with more than 2,000 individual training awards, including doctoral student awards, studentships and new investigator awards, as well as 88 Strategic Training Initiative in Health Research grants that collectively support more than 1,000 trainees.

These grants and awards recognize excellence across the full spectrum of health research, with important results in 2003-04 produced by CIHR-supported researchers in key areas of health research, including:

  • Biomedical research, which investigates the underlying causes of disease and uses animal models to develop new treatments:
    • A virus found in cattle and pigs has been effective in killing cancer cells in mice by growing inside the tumour cells and destroying them. Drs. John Bell and David Stojdl of the Ottawa Regional Cancer Centre say the virus also makes normal cells more resistant and teaches the body to protect itself from a recurrence of the cancer.
  • Clinical research, which takes research the next step from the laboratory to the patient, by testing new diagnostic methods and treatments:
    • A pacemaker seems to provide little help for people who faint frequently due to a problem with their nervous system, according to a CIHR-funded clinical trial carried out by Dr. Stuart Connolly of McMaster University. The trial indicates that pacemaker therapy, an expensive therapy that carries the risk of complications, should not be used in this patient group.
  • Health services and systems research, which focuses on how to strengthen our health care system and help it deliver services to all Canadians, whatever their situation:
    • People with Alzheimer's disease who live in rural or remote areas have limited access to the services they need and their family caregivers are often reluctant to use the services available to them, such as home care and support groups. Dr. Debra Morgan of the University of Saskatchewan has worked with communities and care providers to identify eight barriers to the use of formal services, including the stigma of dementia, lack of privacy and anonymity, lack of awareness and lack of access to services because of distance. The study also proposed strategies to overcome the barriers, including making caregivers aware of available services and how they can help in the particular situation, as well as better public education to break down the stigma around dementia.
  • Population health research, which examines the determinants of health, such as genetics, society, culture and environment:
    • Smoking just one or two cigarettes may be all it takes for some adolescents to become addicted, according to research by Dr. Jennifer O'Loughlin of McGill University. Her study of more than 1,200 Montreal high school students also found that girls are more likely than boys to report symptoms of addiction. The study challenges the current belief that nicotine addiction can take up to three years to develop; as well, it highlights that nicotine dependence could be a stronger factor in why kids smoke than having friends and family members who smoke. The study is the first part of a six-year project examining the genetic and environmental factors involved in nicotine dependence among young people. It underscores the importance of starting prevention and smoking cessation programs earlier and will help in the development of more effective cessation programs.

CIHR support is spread throughout the country, with universities in all ten provinces receiving funding. Funding in all regions of Canada has increased over the past four years, with the Atlantic provinces witnessing some of the greatest percentage increases. Through its funding support, CIHR is catalyzing the growth of an innovative and energized health research community throughout Canada.

CIHR goes beyond geography, however, to bring together health researchers across disciplinary boundaries. A new approach to health research requires a new generation of health researchers with a whole new skill set - the ability to work with colleagues in a wide range of disciplines. Through a number of innovative programs, CIHR is facilitating the development of interdisciplinary teams of young researchers who receive mentorship and training while learning to work with their colleagues from diverse fields. A key feature of many of these programs is partnerships with stakeholders across the country.

In 2003-04, CIHR awarded 33 training program grants, worth $54 million over the next six years, including:

  • The Alberta Cancer Board Training Program in Translational Cancer Research in partnership with the University of Alberta and the University of Calgary, led by Dr. Carol E. Cass of the Alberta Cancer Board, with the involvement of the National Cancer Institute of Canada;
  • IMPACT: Integrated and Mentored Pulmonary and Cardiovascular Training, led by Dr. Peter Paré of the University of British Columbia, with the participation of the Heart and Stroke Foundation of Canada;
  • An Integrated Cancer Research Training Initiative in Nova Scotia, led by Dr. Gerald Johnston of Dalhousie University, in partnership with Cancer Care Nova Scotia and the National Cancer Institute of Canada; and
  • Culture and Mental Health Services, led by Dr. Laurence Kirmayer of McGill University, in partnership with the Fonds de la recherche en santé du Québec.

In the same period, 23 collaborative multi-disciplinary teams were awarded a total of $30 million, including:

  • Health and Aging in Rural Environments, led by Laurel Strain of the University of Manitoba;
  • Rigorous scientific evaluation of selected anti-diabetic plants: Towards an alternative therapy for diabetes in the Cree of Northern Quebec, led by Dr. Pierre Haddad of the Université de Montréal;
  • STRESS: Stress, Trauma and Recovery: Early Stage Studies, led by Dr. Nicholas Coupland of the University of Alberta; and
  • Spinal Cord Injury: Discovery and Development of Rescue and Repair Strategies, led by Dr. Lynne Weaver of the John P. Robarts Research Institute, in partnership with the Ontario Neurotrauma Foundation.

The value of health research: Saving memories

He used to know the names of everybody he met - but now the faces aren't as familiar anymore.

About a year ago, Theo Chester Black began to notice that he couldn't remember things. In July 2003, Mr. Black went to see Dr. Serge Gauthier, a CIHR-funded clinical researcher at the McGill Centre for Studies in Aging at Douglas Hospital, who confirmed Mr. Black's fear: he had early stage Alzheimer's disease. Now Mr. Black, an 80-year-old World War II veteran, is involved in the greatest fight of all: trying to retain the memories of the people and events of his lifetime.

Aricept, a prescription drug, has helped. This commonly-used treatment for Alzheimer's disease helps patients with their memory, thinking, everyday activities and overall behaviour. So far there have been plenty of benefits for Mr. Black.

"Things don't bother me as much as they did," he says. "I'm more alert."

Now, Mr. Black is a candidate for an international clinical trial of a promising new drug, one that, in combination with Aricept, could slow the progression of the disease even more. The drug - so new that it does not yet have a name, just a number - works by stimulating brain repair proteins. If the drug proves effective, it could be added to standard treatments already used to slow down the progression of Alzheimer's disease.

Mr. Black is ready to participate if he qualifies for the trial, and he is delighted that this kind of health research exists for him.

"I don't know where I would be without it," he says.


CIHR
is also committed to easing the administrative burden on researchers, so they can spend more time furthering their research. Through the vision of ResearchNet, a CIHR-led partnership between government and the voluntary sector, it will soon be simpler for researchers to access Canadian research opportunities, apply for and receive research grants and awards and access the latest research information through one central point of entry to all research funding opportunities, regardless of their source. The first of these common services developed for researchers is the Common CV, which can be used to apply to multiple funding agencies. There are more than 23,000 researchers using the Common CV, which is supported by ten provincial, federal and not-for-profit agencies.

Addressing Emerging Health Challenges

Several issues brought Canada's public health system to the forefront in 2003-04 and CIHR demonstrated the importance of health research in responding to these issues.

Health-related crises, primarily SARS, highlighted the importance of being able to respond quickly and effectively to protect the public from the impact of infectious diseases. At the same time, new data underscored the growing importance of obesity as probably the most important public health issue facing not just Canada, but the world.

SARS: A rapid research response

The outbreak of SARS in China, Canada and other countries posed unprecedented challenges to Canada's public health system - and CIHR helped to meet that challenge.

Canadian health researchers made world breakthroughs in investigating the disease. They sequenced the SARS genome in just 11 weeks, setting records for the rapidity of their response. On April 13, 2003, they announced to the world that SARS was associated with a coronavirus. Within months, by December 2003, they had developed three vaccines ready for testing in animals. And researchers in Toronto developed a treatment for SARS, based on a combination of steroids and interferon.

Even though SARS did not reappear in the winter of 2003-04, we can be sure that there will be other, similar outbreaks of infectious disease, brought to our cities by the forces of globalization, international travel and sub-standard public health conditions in many parts of the world. So CIHR-funded researchers extended their focus beyond the disease itself to examine how it spread, how it was contained, what factors in our public health and health care system worked well - and what factors did not.

Since its creation in 2000, CIHR has invested in the study of infectious diseases, including HIV/AIDS, hepatitis C and bovine spongiform encephalitis, or mad cow disease. With the advent of SARS, CIHR took a three-pronged approach:

  • First, CIHR funded, with its many partners in the federal and provincial governments, four research teams in Quebec City, Toronto, Hamilton and Vancouver to understand the severe lung disease caused by the virus, develop a rapid diagnostic test for it, engineer a vaccine and develop antiviral drugs;
  • Second, four of CIHR's Institutes, again working with partners, began funding more research teams to examine and evaluate the public health and health care systems' response to the SARS outbreak; and
  • Third, CIHR formed the Canadian SARS Research Consortium, a unique public-private partnership that is developing an integrated national and, ultimately, international strategy to coordinate, catalyze and fund research on SARS.

The Canadian health research community was able to respond to the SARS outbreak so quickly and comprehensively in no small part due to the important community-building role played by CIHR's Institute of Infection and Immunity. The Institute worked tirelessly to quickly pull together researchers, funders and policy makers to facilitate the research community's response to SARS.

Obesity: A chronic and growing problem

Obesity is a growing problem worldwide, particularly among children. Children who are obese are placing their long-term health at risk. Obesity is a risk factor for heart disease, stroke, type 2 diabetes and fatty liver and gallbladder disease. Unhealthy eating, being overweight and physical inactivity may be responsible for 30% of cancers in the developed world.

That is why CIHR's Institute of Nutrition, Metabolism and Diabetes declared obesity and maintaining a healthy body weight to be its number one - and only - health research priority. And, in January 2004, it invited all Canadians to join in the effort and "donate their steps" to health research. Canada on the Move is a unique partnership involving CIHR and Kellogg Canada in encouraging Canadians to use step counters, or pedometers, to monitor how many steps they take each day and to enter the data at http://www.canadaonthemove.ca/. Health researchers will use that information to identify strategies and factors that encourage a sustained increase in physical activity and reduce obesity.

Health research: revealing the keys to obesity

CIHR-funded researchers are helping to advance knowledge about the causes of obesity, which include both biological and lifestyle components.

  • Dr. Katherine Cianflone of the McGill University Health Centre found that a receptor protein found on fat cells may play a role in obesity. Dr. Cianflone found that the protein, C5L2, binds to ASP, a protein that affects fat production and that people who are obese have in high levels. By identifying molecules that block C5L2 activity, we may be able to slow down the fat-producing process, helping to treat obesity in some people.
  • Lifestyle plays an important role in the incidence of obesity. Watching TV and playing video games increase a child's likelihood of being overweight or obese by anywhere from 10-61%, while sport and physical activity, whether organized or unorganized, decrease the likelihood by 10-43%. Evidence of the link between physical inactivity and childhood obesity comes from a study of children aged 7-11 by Drs. Mark Tremblay of the University of Saskatchewan and J. Douglas Williams of the University of New Brunswick.
  • Obesity can be deadly - one in 10 deaths among adults aged 20-64 is directly attributable to excess weight, according to research conducted by Dr. Peter Katzmarzyk of Queen's University. Obesity-related deaths among this age group have doubled between 1985 and 2000, from 5.3% of all deaths to 9.3%.

Developing a Balanced Research Agenda

Finding a cure for disease - it's guaranteed to get headlines. It requires an intimate understanding of the molecular basis of the disease, how it takes root and how to cut the disease process short - first in the laboratory and then in human patients. But it's equally important to know how to prevent the disease in the first place, by knowing what the risk factors are, who is at risk, and teaching people how to avoid the risks. It is also important to know how best to deliver the cure through our health care system or, in the absence of a cure, how to most effectively treat and deal with the side effects of disease.

CIHR takes a balanced approach to disease, one that encompasses basic biomedical and clinical research, but also research into health services and systems as well as the health of populations. It funds researchers in disciplines ranging from molecular biology to engineering, from bioinformatics to the social sciences and humanities. Through its programs, CIHR encourages researchers from multiple disciplines to work together to bring the benefit of each of their areas of specialization to solve complex health problems requiring a multi-disciplinary approach. This approach is facilitated by CIHR's problem-based Institute structure that brings together researchers across geographic and disciplinary boundaries to focus on the health issues that affect Canadians most.

For instance, CIHR is bringing together members of a number of disciplines, including genetics and population health, to undertake a long-term study that will examine the impact of genetics, biology, environment and lifestyle on children's health. The study, which will require a major investment to follow the children for up to 20 years, was in its planning phase in 2003-04.

Cardiovascular disease provides another example of the balanced approach CIHR takes to research. It is the leading cause of death in Canada, accounting for at least 36% of all deaths, or about 80,000 people each year. More than 450,000 Canadians were hospitalized for cardiovascular disease in 2000. Cardiovascular disease is also the costliest disease affecting Canadians, responsible for $18.5 billion in expenditures in 1998, 11.8% of the total cost of all illness in Canada.

The first step in preventing heart disease is mapping who is most affected. In Canada, Atlantic Canadians have the highest rate of risk factors for heart disease and the highest risk of dying of it, according to a new atlas of cardiovascular disease in Canada released in 2003-04 by the CIHR-funded Canadian Cardiovascular Outcomes Research Team (CCORT).

By identifying geographic areas at highest risk, the team hopes to encourage policy makers to develop programs to help residents avoid heart disease, such as aggressive anti-smoking campaigns.

Equally important is treating those who have heart disease and ensuring they receive the best possible treatments. A major CIHR-funded clinical trial conducted several years ago showed that a drug called Ramipril reduced the risk of heart attack by 25% in high-risk patients and improved survival after heart attack. Now, as reported in October 2003, research carried out by CCORT has found that prescriptions for Ramipril increased by more than 400% in Ontario in the wake of that trial, demonstrating the major impact a clinical trial can have on drug prescribing and utilization.

Surviving a heart attack depends to a large degree to the quality of care received in hospital. In their study of cardiac care in Ontario, Dr. Jack Tu, leader of CCORT, and his team found that as many as 400 lives could be saved each year in Ontario if more heart attack and heart failure patients received needed medications before leaving hospital. The difference, according to researcher Dr. Philip Jong of the University of Toronto, may be care by a cardiologist. He found that cardiologists were more likely than other physicians to prescribe heart failure medication to patients when they were discharged from hospital. This resulted in a lower one-year mortality rate for their patients than for patients who were treated by other physicians (28% compared to 36%). This research is an alert to physicians that they can improve outcomes for their patients by using the latest evidence-based therapies.

Who is Jack Tu and why does he know so much about heart disease?

Dr. Jack Tu knows more about cardiovascular disease in Canada than probably any other researcher. That's because he is at the head of the Canadian Cardiovascular Outcomes Research Team (CCORT), a partnered research project with the Heart and Stroke Foundation of Canada and one of 16 Interdisciplinary Health Research Teams funded by CIHR.

Dr. Tu has brought together a team of 25 researchers from Nova Scotia, Quebec, Ontario, Alberta and British Columbia to improve the quality of cardiovascular care in Canada. Seven different projects share one common theme - using health care "report cards" to improve the quality of care.

To date, the project has produced important results that will help physicians provide better treatment to their patients while providing policy makers with the information they need to ensure a consistent quality of care for all people with cardiovascular disease.

No one approach to a pervasive disease such as cardiovascular disease will succeed in reducing its toll. Rather, we need to focus on understanding who is at risk and how to minimize their being afflicted by the disease, how to treat it most effectively when it occurs and how the health care system can be structured to best respond. Only with this balanced approach can we hope to reduce the toll from this debilitating disease, both in terms of productive lives cut short and escalating costs to the health care system.

Face to face and heart to heart

Maxine Vukic has seen something few of us ever will - her own heart.

In August 2001, still in her twenties, Ms. Vukic had a heart transplant. And while her new heart is beating in her chest, her old heart is being analyzed by researchers at the University of British Columbia.

The transplant surgery was the end of a long road that started in early 1996, when Ms. Vukic was diagnosed with an undisclosed heart virus, and ended in transplant surgery in August 2001.

"It was a really long, painful thing to go through," she remembers.

Now, researchers at UBC's iCAPTURE4E Centre are analyzing her heart to find out what went wrong. After her surgery, Ms. Vukic was given a tour of the Centre - and came face to face with her own heart.

"I was standing in front of my diseased heart," she says, "with my new heart beating inside of me. It was really weird to connect the two together."

Ms. Vukic says her old heart looked "rather sick," but is pleased that it could contribute to helping others.

"If researchers can figure out what happened to my heart and prevent it from happening again," she says, "that's definitely worth it. Their work will save other people from going through my experience.

Improving the Health of Vulnerable Populations

In a country with universal access to health care, all citizens should enjoy equal opportunities for health. The reality, however, is that many Canadians, including people living in poverty, those living in rural or remote areas, the homeless, immigrants, refugees and many others, do not have the same access to health services as other Canadians and do not enjoy the same health status. For instance, Canada's Aboriginal Peoples have lower life expectancies, higher infant mortality rates and higher rates of HIV infection and diseases such as diabetes and tuberculosis.

Since so many of the determinants of health, such as income, education and housing, lie outside the health care system, reducing these inequities is a complex process that requires the dedication of many different disciplines and sectors. CIHR is committed to supporting research that will improve the health status of Canada's vulnerable populations. In September 2003, in partnership with the National Secretariat on Homelessness, CIHR announced an investment of more than $1.2 million for research to reduce health disparities and improve the health of vulnerable populations.

This goal must be achieved in collaboration with these populations. Many Aboriginal communities, for example, have had negative experiences in the past with health researchers who arrived without consulting the community and left without explaining the findings of their research.

Today, to redress those past experiences, the CIHR Institute of Aboriginal Peoples' Health is working in close collaboration with communities. To help ensure that health research is carried out with the full support and benefit of communities, the Institute began a process in 2003-04 to make explicit a detailed code of research ethics that articulates realistic roles and expectations for both researchers and communities. As well, during 2003-04, it funded six community-based health research projects, projects that build close partnerships between scientists and communities and involve communities at every stage of the research.

The community-based projects are breaking down barriers between researchers and Aboriginal communities by training health researchers who come from Aboriginal communities. An advisory board with a majority of Aboriginal members ensures that an Aboriginal viewpoint is included and that the needs of the community are respected. There are now eight Aboriginal health research centres across the country (in British Columbia, Alberta, Saskatchewan, Manitoba, Ontario, Quebec and Nova Scotia), supporting 54 undergraduate students, 26 master's students, 21 doctoral candidates and 5 post-doctoral fellows. Dr. Éric Dewailly, a noted researcher on the impact of western lifestyles on the Inuit, is leading the Centre for Inuit Health and Changing Environments, based in Quebec City.

Rural and northern residents are also vulnerable by virtue of their distance from many of the health services and practitioners that urban residents take for granted. CIHR is investing $500,000 in projects that address the health needs of rural and northern residents. Among the recipients of funding is Dr. Judith Guernsey of Dalhousie University, who is studying the unique health challenges facing rural and remote Canadians, focusing largely on Atlantic Canada.

Concern for vulnerable populations extends beyond Canada's borders, to encompass all countries of the world. CIHR is helping to address the 10/90 Gap, whereby only 10% of the $73.5 billion (US) that was invested in health research worldwide in 1998 was allocated to the health problems of 90% of the world's population. In March 2004, CIHR presented its Distinguished Investigator Award to Dr. Peter Singer of the University of Toronto's Joint Centre for Bioethics. Dr. Singer is one of Canada's leading bioethicists, specializing in the fields of bioethics and global health. His research develops and implements policy interventions to help prevent a 'genomics divide' between industrialized and developing countries. The science of genomics promises to revolutionize health-related sciences, which have been energized by the human genome project and the knowledge it is generating. Dr. Singer wants to harness the potential benefits of genomics in order to improve global health equity.

United against Diabetes

In the 1980s, Mohawk elders in the community of Kahnawake, Quebec, became concerned about high rates of diabetes in community members and decided that focusing on children was the answer. The Kahnawake Schools Diabetes Prevention Project (KSDPP), formed in 1994, focused on reducing the behaviours that were leading to diabetes, especially diet and exercise.

The health outcomes - reversing the growing incidence of diabetes - will take time to achieve. But KSDPP has had the immediate benefit of involving the entire community in making a commitment to living more healthy lives, by understanding the links between lifestyle and disease and planning and implementing research that will have tangible results.

KSDPP offers cooking classes and harvest fairs to promote healthy eating. It sponsors winter carnivals, soap box derbies, Mohawk Miles and other family events to encourage physical activity. And it conveys the dangers of diabetes to other Aboriginal communities through the Wampum Walk, which was created by an elder in 1997 to revive an ancient First Nations tradition of communicating with neighbouring nations. A wampum belt with a symbolic message ("our blood is too sweet") has been carried on foot, bicycle and by canoe to share the message.

Thanks in part to five-year funding from CIHR, the KSDPP Centre for Research and Training in Diabetes Prevention, under the leadership of Dr. Ann Macaulay, now shares the KSDPP model with other communities.

Supporting Health Innovations

There has been remarkable progress over the past decade in our fundamental understanding of human biology. Today, the challenge is to convert these discoveries into innovations that will lead to new ways to maintain a healthy population and prevent disease, as well as diagnose and treat it when it occurs. We need to ensure that where there is evidence to inform decisions, we use it to strengthen our health care system. The process of transforming knowledge into action is complex; nonetheless, it is essential if Canadians are to realize the exceptional value of federally funded health research.

CIHR is unique in that its mandate calls upon the organization to translate new knowledge into "improved health for Canadians, more effective health services and products and a strengthened Canadian health care system". Throughout 2003-04, CIHR continued to support research on knowledge translation (KT), to learn more about the most effective strategies and techniques to disseminate and exchange knowledge created through health research.

Dr. Jeremy Grimshaw of the University of Ottawa is a world-leading expert in knowledge translation and one of two recipients of CIHR's first Knowledge Translation Award. Dr. Grimshaw came to Canada from Scotland, attracted by the outstanding support he could receive to move his research forward. His research focuses on four key areas: systematic reviews of interventions to improve health service delivery and systems; evaluation of KT strategies; development of a theoretical basis for KT; and a situational analysis of Canadian KT activities.  Known for his creativity, innovation and uniqueness and for his commitment to high scientific standards, Dr. Grimshaw is assisting health researchers across the country to transform their research into practice.

Occupational Health and Safety Agency for Healthcare

The Occupational Health and Safety Agency for Healthcare, under the leadership of Executive Director Dr. Annalee Yassi, has helped to reduce British Columbia's injury rate for the health care industry by 27% and annual days lost per 100 full-time employees by 38% since 1999. It has succeeded by combining world scientific knowledge with local knowledge to identify and implement best practices.

The Agency was created by health care unions and employers working together to combat skyrocketing injury rates among health care workers. It currently has more than 50 studies underway, focusing on topics ranging from preventing exposure to blood-borne pathogens and SARS to violence prevention.

The Agency also leads a CIHR Community Alliance for Health Research project initiated by Dr. Yassi entitled "Making Healthcare a Healthier Place to Work: A Partnership of Partnerships". For its accomplishments, the Agency was one of the first two recipients of CIHR's Knowledge Translation Award, in March 2004.

Many of the bacteria that cause infections in hospitals are antibiotic-resistant. Dr. Neil Reiner of the University of British Columbia is addressing this issue and has discovered a strategy for drug targeting that could lead to new, more effective antibiotics for hospital-acquired infections and many infectious diseases.

He and his colleagues have found that certain essential proteins that are present in both microbes and humans are remarkably similar - yet different in key areas. With this information, the team is targeting these areas of distinctness, in the belief that there is less chance of toxicity and greater likelihood of effectiveness. Testing to date has shown that targeting these proteins can have an antibiotic effect. He is now using CIHR support to test this new strategy with candidate drugs.

One in 20 Canadians over age 65 and one in four over age 85 is affected by Alzheimer's disease. The cognitive decline that occurs as a result of this and other neurodegenerative diseases is linked to amyloid deposition. Dr. JoAnne McLaurin of the University of Toronto has developed small carbohydrate-based molecules that inhibit amyloid deposition, reducing cognitive decline. She has shown that the molecule works in animal models.

Now, with support from CIHR, she is working with Toronto's Ellipsis Biotherapeutics Corporation to carry out the toxicology tests that are a necessary precursor to human clinical trials. If these are successful, physicians will have a new weapon in the battle against Alzheimer's and other neurodegenerative diseases.

The marketplace is also a key means of knowledge translation, making new health products and services available to Canadians. During 2003-04, CIHR expanded its commercialization strategy to catalyze innovation and move health research from the academic setting to the marketplace. With the strategy's three streams of mobilizing research, developing people and building partnerships, CIHR is building on its current suite of programs to encourage innovation. This will result in improved health for Canadians, more effective health services and products and a strengthened economy.

CIHR's innovative programs help transform today's health research discoveries into tomorrow's health care solutions. They encourage the development of innovative new diagnostic methods and therapies that save lives and reduce the economic burden of ill health.

Providing Stewardship and Accountability

CIHR reports to Parliament through the Minister of Health. Its Governing Council is chaired by CIHR's President and comprises 20 Canadians who have been appointed by Order-in-Council to renewable three-year terms. Management Discussion and Analysis

Fiscal year 2003-04 was CIHR's fourth year of operation since its creation by legislation in June 2000. Since that time, CIHR's budget has increased by $328M.

In 2003-04, CIHR funded 8,254 grants and awards for a total investment of $646.9M. This includes the funding of 11 Networks of Centres of Excellence and 351 Canada Research Chairs.

CIHR's operating expenditures in 2003-04 totalled $41.3M, of which $23.5M (57%) was for salary and employee benefits and $17.8M (43%) for non-salary expenditures.

Highlights

  • Overall growth in CIHR's budget was 11.9% over 2002-03.
  • Participation in strategic initiatives by partners from government, the voluntary and the private sectors attracted $76.7M in additional funding for grants and awards.
  • CIHR's Institute-led Requests for Applications (RFAs) for the funding of strategic initiatives increased from 48 in 2002-03 to 84 in 2003-04.
  • The number of grants and awards increased to 8,254 in 2003-04 from 7,504 in 2002-03, an increase of 10%.
  • The ratio of operating expenditure to total budget has been maintained at less than 6%.
  • The Operating Budget incurred a surplus of $5.987M, or about 12% of Budget. The amount that will be carried forward into fiscal 2004-05 will total approximately $2M.
  • In the CIHR Grants Budget a significant lapse in funds earmarked for the Canada Research Chairs Program (CRC) was forecast. Because of this, the Treasury Board of Canada placed $32M of CIHR's Grants Budget into a frozen allotment that was allowed to lapse, as a contribution to the Government's expenditure reduction program. The nature of this program is such that there will be no impact on the capacity of CIHR to fund Chairs in the health component of the program in subsequent years. The remaining lapse in the Grants Budget is also primarily attributable to the Canada Research Chairs Program.

Outlook 2004-05

CIHR continues to focus on achieving an appropriate balance between open competitions and strategic initiatives. The goal is to ensure that CIHR funding programs are well positioned in the future to address specific health research priorities and opportunities. Maintaining this balance however, increases the complexity of financial planning for
2004-05 and beyond.

Health research is a long-term endeavor that requires multi-year financial commitments. Thorough financial planning is required to manage risk associated with potential differentials between commitments to grants and awards and the funding expected from Parliament.

In its February 2004 Budget, the federal government announced an additional $39M in annual funding for CIHR in fiscal year 2004-05 and beyond. This will enable CIHR to further increase its investments in strategic initiatives, including programs in commercialization and knowledge translation.

Peer Review

The peer-review system ensures that CIHR's funding process is fair and open, that taxpayers' money is spent wisely and that only the best and brightest researchers are funded. Each year, CIHR receives thousands of funding applications. Each application is evaluated by a peer-review committee composed of a number of volunteer reviewers who write detailed reports on its strengths and weaknesses. Through a process of consensus, the committee arrives at a numerical rating for each proposal. As a result, only those that meet internationally accepted standards of excellence are funded.

The peer-review process is the backbone upon which CIHR's reputation for excellence rests. Peer reviewers are accountable to the wider research community and help to ensure that CIHR is accountable to the Government of Canada for the money it invests in health research. CIHR recognizes the dedication of over 2,575 experts who volunteered their time on one of CIHR's peer-review committees during the period from April 1, 2003 to March 31, 2004 and thanks them for their continued contribution to improving the lives of Canadians. CIHR also appreciates the efforts of thousands of external referees who submitted written reports for consideration by the peer-review committes.

Management responsibility for financial statements

Responsibility for the integrity and objectivity of the accompanying financial statements of the Canadian Institutes of Health Research for the year ended March 31, 2004, and all information contained in this report rests with CIHR's management.

These financial statements have been prepared by management in accordance with Treasury Board of Canada accounting standards based on Canadian generally accepted accounting principles. Where appropriate, the financial statements include amounts that have been estimated according to management's best judgement. These statements should be read within the context of the significant accounting policies set out in Note 2 of the financial statements.

Management has developed and maintains books of accounts, records, financial and management controls and practices and information systems. They are designed to provide reasonable assurance that CIHR's assets are safeguarded and controlled, that resources are managed economically and efficiently in the attainment of corporate objectives and that transactions are in accordance with the Financial Administration Act and regulations as well as CIHR policies and statutory requirements.

The transactions and financial statements of CIHR have been audited by the Auditor General of Canada, the independent auditor for the Government of Canada.

To the Canadian Institutes of Health Research
and the Minister of Health

I have audited the statement of financial position of the Canadian Institutes of Health Research (CIHR) as at March 31, 2004 and the statements of operations and net assets and cash flow for the year then ended. These financial statements are the responsibility of CIHR's management. My responsibility is to express an opinion on these financial statements based on my audit.

I conducted my audit in accordance with Canadian generally accepted auditing standards. Those standards require that I plan and perform an audit to obtain reasonable assurance whether the financial statements are free of material misstatement. An audit includes examining, on a test basis, evidence supporting the amounts and disclosures in the financial statements. An audit also includes assessing the accounting principles used and significant estimates made by management, as well as evaluating the overall financial statement presentation.

In my opinion, these financial statements present fairly, in all material aspects, the financial position of CIHR as at March 31, 2004 and the results of its operations and its cash flow for the year then ended in accordance with Canadian generally accepted accounting principles.

Notes to the financial statements
For the year ended March 31, 2004

1. Authority and Objectives

The Canadian Institutes of Health Research (CIHR) was established in June 2000 under the Canadian Institutes of Health Research Act, replacing the former Medical Research Council of Canada. It is listed in Schedule II to the Financial Administration Act as a departmental corporation. CIHR's objective is to excel, according to international standards of scientific excellence, in the creation of new knowledge, and its translation into improved health, more effective health services and products, and a strengthened Canadian health care system.

CIHR is led by a President who heads a Governing Council of nineteen other eminent Canadians appointed by Order in Council. The Governing Council sets overall strategic direction, goals and policies and oversees programming, resource allocation, ethics, finances, planning and accountability.

CIHR operates a wide variety of grants and awards programs to support health research, develop researchers, build a robust health research environment, promote partnerships, engage the public and foster use of research results.

CIHR has 13 Institutes that focus on identifying the research needs and priorities for specific health areas, or for specific populations, then developing strategic initiatives to address those needs. Each Institute is led by a Scientific Director who is guided by an Institute Advisory Board, which strives to include representation of the public, researcher communities, research funders, health professionals, health policy specialists and other users of research results.

CIHR strives to support the full spectrum of health research - biomedical, clinical, health services and population health - and recognizes that the complexity of many health issues requires an integration of the perspectives and research approaches of different health disciplines.

The entire CIHR program, administration excepted, is achieved through transfers in the form of grants for research projects, personnel awards and institute support grants.

2. Significant Accounting Policies

These financial statements have been prepared in accordance with Treasury Board of Canada accounting standards based on Canadian generally accepted accounting principles. The most significant accounting policies are as follows:

(a) Parliamentary appropriations - CIHR is financed by the Government of Canada through Parliamentary appropriations. Appropriations provided to CIHR do not parallel financial reporting according to generally accepted accounting principles. They are based in a large part on cash flow requirements. Consequently, items recognized in the statement of operations and the statement of financial position are not necessarily the same as those provided through appropriations from Parliament. Note 10 provides a high-level reconciliation between the two bases of reporting.

(b) Net cash provided by government - is the difference between all cash receipts and all cash disbursements including transactions between departments.

(c) Due from the Consolidated Revenue Fund - all departments, including agencies and departmental corporations, operate within the Consolidated Revenue Fund (CRF). The CRF is administered by the Receiver General for Canada. All cash receipts are deposited to the CRF and all cash disbursements made by departments are paid from the CRF. Due from the CRF represents the amount of cash that CIHR is entitled to draw from the Consolidated Revenue Fund without further appropriations, in order to discharge its liabilities.

(d) Revenues - these are accounted for in the period in which the underlying transaction or event occurred that gave rise to the revenues.

(e) Deferred revenue - monies received as donations and contributions from various organizations and individuals for health research as well as interest on endowments are recorded as deferred revenue until such time that they are disbursed in accordance with agreements between the contributor and CIHR or in accordance with the terms of the endowments.

(f) Expenses - these are recorded when the underlying transaction or expense occurred as follows:

  • Grants and awards are recognized in the year in which payment is due or in which the recipient has met the eligibility criteria.
     
  • Employee severance benefits are accrued as earned and are calculated using information derived from the results of the actuarially determined liability for employee severance benefits for the Government as a whole. Employee severance benefits on cessation of employment represent obligations of CIHR that are normally funded by appropriation when the benefits are paid.
     
  • Vacation pay and compensatory benefits are expensed in the year that the entitlement occurs.
     
  • Contributions to superannuation plans are recognized in the period that the contributions are made. Actuarial surpluses or deficiencies are not recorded in CIHR's accounts but are recognized in the consolidated financial statements of the Government of Canada.
     
  • Services provided without charge by other government departments and agencies are recorded as operations and administration expenses at their estimated cost.

(g) Accounts receivable - these are stated at amounts expected to be ultimately realized. A provision for doubtful accounts is made for any amounts where recovery is considered uncertain.

(h) Capital assets - all tangible assets having an individual initial cost of $5,000 or more are recorded at their acquisition cost. Amortization of capital assets is done on a straight-line basis over the estimated useful life of the capital asset as follows:

(i) Foreign currency transactions - transactions involving foreign currencies are translated into Canadian dollar equivalents using rates of exchange at the time of those transactions.

(j) Refunds of previous years' expenses - these relate to grants and awards and are recorded as a reduction in expenses when received.

(k) Measurement uncertainty - the preparation of financial statements in accordance with Canadian generally accepted accounting principles requires management to make estimates and assumptions that affect the reported amounts of assets, liabilities, revenues and expenses reported in the financial statements. At the time of preparation of these statements, management believes the estimates and assumptions to be reasonable. The most significant items where estimates are used are allowances for employee vacation and compensatory benefits, allowance for employee severance benefits and amortization of capital assets.

4. Deferred Revenue

5. Net Assets

Included in the Net Assets are two endowments for health research. These endowments are restricted assets that cannot be spent. The interest on these accounts is credited to deferred revenue.

6. Contingent Liabilities

A legal suit for employment equity was initiated by the Public Service Alliance of Canada against Her Majesty the Queen naming certain separate employer organizations of the Government of Canada, including the Canadian Institutes of Health Research (CIHR), as defendants. The amount of this claim, as it relates to CIHR, is estimated to be $750,000. In management's opinion, the outcome of this litigation is not presently determinable.

Two other legal suits launched by individuals alleging damage from participation in projects funded by grants from the Medical Research Council of Canada are pending. The amount of these claims is estimated at $50,000. In management's opinion, the outcome of this litigation is not presently determinable.

7. Commitments

The Canadian Institutes of Health Research is committed to disburse grants and awards in future years subject to the provision of funds by Parliament. Future year commitments are as follows.

8. Open Competitions and Strategic Initiatives

Schedule 1 displays CIHR's grants and awards programs. Canadian health researchers may compete for grants and awards from these programs through two funding mechanisms. Open competitions refer to competitions in each of these programs, which do not relate to any specific area of scientific inquiry. Peer review ranks the scientific merit of each application and the top ranked applications are funded regardless of which area of science they represent. Strategic Initiatives refer to competitions aimed at supporting research in very specific areas of science or for developing research capacity in specific segments of the Canadian research enterprise.

9. Services provided without charge by other Government departments

CIHR is related in terms of common ownership to all Government of Canada departments, agencies, and Crown corporations. CIHR enters into transactions with these entities in the normal course of business and on normal trade terms applicable to all individuals and enterprises except that certain services, as shown below, are provided without charge.

Since CIHR had forecast a significant lapse in funds earmarked for the Canada Research Chairs Program (CRC), the Treasury Board of Canada required CIHR to lapse 32 million dollars in Vote 15 as a contribution to the Government's expenditure reduction program. The remaining lapse in Vote 15 is also attributable to the CRC Program. The lapse in funds does not have an impact on CIHR's capacity to fund CRCs in subsequent years.

11. Employee future benefits

Employees of CIHR are entitled to specific benefits on or after termination or retirement, as provided for under various collective agreements or conditions of employment.

(a) Pension benefits

Employees participate in the Public Service Superannuation Plan administered by the Government of Canada. Under present legislation, contributions made by CIHR to the Plan are 2.14 times the employee's contributions on account of current services. CIHR's contributions to the Plan during the year amounted to $1,732,450 (2003 - $1,329,552).

(b) Severance benefits

CIHR provides severance benefits to its employees. This benefit plan is not pre-funded and therefore has no assets, resulting in a plan deficit equal to the employee severance benefits liability. Information about the plan is as follows:
